• Appointment details and scheduling information submitted through Heal.me

• Information submitted through GoHighLevel forms, surveys, chat, email, or text

• Any information you voluntarily provide related to wellness goals, habits, or concerns

Information collected automatically

• IP address, browser and device information, and general site usage data

• Cookies or similar technologies used for functionality and analytics

2. HOW WE USE INFORMATION

We may use information to:

• Schedule, deliver, and administer services and appointments

• Send confirmations, reminders, and service-related messages

• Respond to inquiries and provide customer support

• Improve the Sites and client experience

• Send marketing communications if you opt in (you can opt out anytime)

• Maintain internal records and comply with legal obligations

3. TEXT MESSAGES (SMS)

If you provide your phone number, you consent to receive service-related texts from us and/or through Heal.me and GoHighLevel, including appointment confirmations, reminders, scheduling updates, and administrative messages. Message and data rates may apply. You can opt out by replying STOP (or using the opt-out method provided in the message). Opting out may limit time-sensitive scheduling communications.

4. THIRD-PARTY PLATFORMS

We use third-party platforms that may process your information, including:

• Heal.me (scheduling and related communications)

• GoHighLevel (forms, CRM, messaging, and marketing automation)

Information you submit through third-party platforms is also subject to their terms and privacy practices.

5. PAYMENTS

Payments may be processed through Square and may be initiated or facilitated through GoHighLevel and/or Heal.me. We do not receive or store full payment card numbers. We may receive limited transaction details such as payment confirmation, date, and amount, depending on the processor.

6. HOW WE SHARE INFORMATION

We do not sell your personal information. We may share information:

• With vendors/service providers who help operate the Sites and services (hosting, scheduling, communications, analytics)

• If required by law or to protect rights, safety, and security

• In connection with a business transfer, subject to appropriate safeguards

7. VIRTUAL COMMUNICATIONS AND SECURITY

Electronic communications (email, text, video, online forms) involve privacy and security risks. We use reasonable safeguards but cannot guarantee absolute security. You are responsible for keeping your devices and accounts secure.

Chris Eve is not a physician. Services are provided as wellness coaching and educational support and do not include medical diagnosis, medical treatment, or medical advice.

2. EDUCATIONAL RELATIONSHIP ONLY

Your relationship with Chris Eve is strictly educational in nature and does not create a physician-patient relationship or other licensed healthcare relationship.

3. MODALITIES AND EDUCATIONAL USE (QEST4, NRT, HSR, HOMEOPATHY)

Services may include educational wellness coaching and non-medical modalities such as Qest4, Nutrition Response Testing (NRT), Heart Sound Recorder (HSR), and homeopathy education. These modalities are not medical tests and are not a substitute for medical evaluation, diagnosis, or treatment. Any results, observations, or reports are not diagnostic.

Just a reminder that these results are energetic in nature and intended for educational purposes.

4. NO GUARANTEES

Individual outcomes vary. We do not guarantee results.

5. EMERGENCIES

Do not use this website or services for emergencies. If you believe you may be experiencing a medical emergency, call 911 or go to the nearest emergency room.

6. VIRTUAL SERVICES

Virtual sessions and electronic communications may be interrupted or involve security risks. By choosing virtual services, you accept these risks and agree to participate from a private, safe location.

7. PERSONAL RESPONSIBILITY

1. USE OF THE SITES 

You agree not to: 

• Use the Sites for unlawful purposes 

• Attempt unauthorized access to systems or data 

• Interfere with the security or operation of the Sites 

2. SERVICES ARE EDUCATIONAL 

All services, content, and communications are educational and not medical care. You agree you are responsible for your own health decisions and will consult a licensed healthcare professional for medical concerns. 

This includes any educational discussion or use of non-medical modalities such as Qest4, Nutrition Response Testing (NRT), Heart Sound Recorder (HSR), and homeopathy education, which are not medical tests and do not diagnose or treat disease. 

3. SCHEDULING, COMMUNICATIONS, AND PLATFORMS 

Scheduling, forms, reminders, and communications may be handled through Heal.me and/or GoHighLevel. Your use of those platforms may also be subject to their terms and policies.

5. PAYMENTS AND POINT OF SALE (POS) 

Payments may be processed through Square and may be initiated or facilitated through GoHighLevel and/or Heal.me. We do not store full payment card numbers on our systems. Payment information is handled by the applicable payment processor in accordance with its policies. We may receive limited transaction details such as payment confirmation, date, and amount. 

If refunds are offered, they are governed by the cancellation/refund terms disclosed at the time of booking and/or purchase. 

6. PAYMENT AND INTAKE DEADLINES 

You agree to the payment and intake requirements disclosed at booking and/or in appointment confirmations, including any deadlines for submitting intake paperwork and making payment prior to an appointment. Failure to meet required deadlines may result in cancellation or rescheduling, as disclosed at booking. 

7. CANCELLATIONS AND NO-SHOWS 

Cancellation/rescheduling and no-show policies are disclosed at booking and/or in appointment confirmations and are incorporated into these Terms by reference. 

8. VIRTUAL VISITS 

Virtual sessions depend on technology and may be interrupted. You agree to participate from a safe, private location and to maintain your own device and internet access. 

9. TESTIMONIALS AND REVIEWS 

Testimonials reflect individual experiences and are not guaranteed outcomes. Any authorization to use testimonials for marketing purposes will be handled through separate written permission and/or intake documentation. 

10. INTELLECTUAL PROPERTY 

All content on the Sites is owned by Chris Eve or used with permission and may not be copied, reproduced, distributed, or used to create derivative works without written permission. 

11. LIMITATION OF LIABILITY 

To the fullest extent permitted by law, Chris Eve is not liable for any direct or indirect damages arising from your use of the Sites, content, or services. 

12. INDEMNIFICATION 

You agree to indemnify and hold harmless Chris Eve from claims and expenses arising from your misuse of the Sites or violation of these Terms. 

13. GOVERNING LAW (ARIZONA) 

These Terms are governed by the laws of the State of Arizona, without regard to conflict of law principles. 

14. CHANGES 

We may update these Terms at any time by posting the revised version with an updated effective date.
